1. To qualify for the program, your team must include at least one UW–Madison student, staff, or faculty member, or employee of an official UW–Madison affiliate organization.
2. All information entered on this application will be treated confidentially. As UW–Madison employees, applicant reviewers will not disclose applicant names, partner names, or application details to anyone not on the D2P staff without your permission.
